A new opening for a Cardiac Nurse Navigator is now open in a riverside town in central Washington. They are hiring a dynamic nurse leader and educator to ensure patient care and services.

The Cardiac Nurse Navigator will work alongside the physicians and interdisciplinary team facilitating patient involvement in the individualized care plans. As the Cardiac Nurse Navigator, you will coordinate all aspects of care immediately from diagnosis to treatment and continue through the post- operative period. This Cardiac Nurse Navigator creates home care plans, schedules, and meetings to ensure effective continuity of care.
