Position Overview:

The LPN will coordinate all medical/psychiatric care and medications for residents ensuring proper documentation, follow-up and regulatory compliance. Assist with hands-on training of residential staff to ensure this compliance is ongoing and current. Secondary responsibilities include resident care and oversight in AOL's. Identify and organize appointments for week in conjunction with RN and Manager

Transports and accompanies residents on as scheduled medical appointments and completes all necessary follow-up upon return from appointment

Performs delegated nursing tasks as directed by the RN and under the supervision of the RN

Oversee coordination of all medication administration including verifying prescriptions, ordering medications, regulatory documentation, and staff administration of medications in conjunction with RN and under supervision of RN

Maintain medication inventory including prescription medications, over the counter medication and required first aid supplies are in stock at all times and that check expiration dates are routinely checked

Ensures maintenance of all medically related documentation (MAR, MD Consults, etc.)

Updates RN daily on resident's medical status

Responsible to complete all documentation prior to end of shift and report all issues/concerns to RN and Residence Manager

Completes the documentation of Occurrence Reports as required

Reviews and makes daily entries into House Log Book on all pertinent resident/residence related issues

Monitors resident progress and notifies applicable others of concerns, changes in behavior or of progress

Responsible to implement all directives issued by RN and Manager as needed

Handles telephone calls and inquiries; provides necessary follow-up; keeps family and management notified when necessary

Knows and applies emergency procedures in event of a crisis including fire safety

Acts as liaison between Residence, Day Program, Physicians and families on all resident medical related issues

Provides individual residents instruction and/or assistance with daily living skills and rehabilitation or social skills in accordance with Individual Service Plan

Provides ongoing training to residents and staff in areas of need identified in Individual Service Plan in conjunction with RN & Manager

Provides hands-on client specific training to staff members as directed by RN

Attends annual and semi-annual reviews providing medical input in absence of RN involvement

Visits residents in hospital, as needed and coordinate Discharge Plan in conjunction with RN

Completes necessary residential habilitation notes, progress notes and other required documentation

Complies with OSHA regulations and universal precautions, AMAP Training, annual recertification training and annual PPD clearance as per agency policy

Attends various meetings/in-services, participates in staff meetings in conjunction with mandatory annual in-services as directed by Residence Manager I RN

Supervises residents' intake of medication as prescribed in accordance with OMRDD approved Medication Administration Procedure

Maintains friendly and professional working atmosphere

Maintains professional appearance

Ensures residents are always treated with dignity

Schedule: Monday- Friday 7a-3p/3p- 11p and alternate weekends

Qualifications:

Education: New York State License Nurse

Experience: Specialized training in I/DD and/or one year experience in community health care services, excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills; strong team building skills; computer skills and knowledge of word processing; demonstrated ability to work effectively with men and women of diverse races, ethnicities, ages and sexual orientations in a multicultural environment.

SPECIAL SKILLS:

CPR and First Aid Trained

Must be a team player in order to interact with Administrative and Managerial staff as well as Supervisory and Line Staff, as appropriate

Have a strong sensitivity to cultural differences in order to work well with staff, individuals and families served.

Able to set limits, maintain role, work collaboratively and respectfully
