Private Duty Registered Nurse (RN)

Job Description
Summit Home Care is a family-owned and operated home care agency that services service Brooklyn, Bronx, Queens, Staten Island, Manhattan, Nassau & Suffolk County & the Lower Hudson Valley.

We are currently seeking exceptional Private Duty RNs to join our team of dedicated professionals. We have multiple shifts open with our clientele that need immediate coverage; nights, days, and weekend hours for 1:1 in-home care.

While we look for experience with GT, Trach & Vents we also offer a phenomenal education program that offers extensive training to new grads looking to work with GT, trach, or vent clients.

Responsibilities:

• Follow a plan of care designed by our Clinical Director
• Perform Medication Administration and Treatments per POC
• Monitor clients conditions and report accordingly
• Accurate documentation in accordance with the provided orders

Requirements:

• Valid NYS RN license
• Valid BLS
• Physical with PPD or Quantiferon (cannot be older than 6 months)
• If the PPD is positive then a chest x-ray is required
• Drug screen test (cannot be older than 6 months)
• Rubella & Rubeola blood work (from the lab)
• Two (2) typed references
• Social security card
• Valid ID (NY State ID / Passport / Resident Card
