Job Description
NSO is looking for PASRR Nurse Evaluators on a Fee-For-Service basis. The FFS evaluator could work in either Oakland and/or Wayne Counties supporting NSO and our great Adult Services program.

The PASRR Nurse Evaluator is a healthcare/clinical position which involves the provision of physical and psycho-social assessments to individuals pursuant to PASRR regulations, as well as the provision of physical and psycho-social assessments to community-dwelling elderly who are clients of NSO-Adult Services

Evaluation services and the provision of outreach physical health care evaluations to clients in hospitals, nursing homes, and other types of residences. Services include physical health assessments, evaluation of current care, completion of PASRR summary for physical examination and medication history.

Clinical interventions are directed toward maintaining clients within least-restrictive settings and toward maximizing capacities for independent functioning.

  • As indicated, provides direct clinical services to consumers with a frequency consistent with good nursing practice and within the Unit standards.
  • Reviews PASRR record, client record, physical exam findings, and makes recommendations for level of care appropriate to the needs of clients.
  • Appropriately utilizes internal resources, such as psychiatric and supervisory consultation, to resolve mental health crises and other problematic situations.
  • Appropriately utilizes outside resources such as hospitals, primary care physicians, etc., to resolve physical health problematic situations in least-restrictive settings appropriate to the consumers nursing needs and as directed.
  • Seeks supervisory consultation when appropriate action is unclear or beyond routine nursing functioning.
  • Relates to evaluated persons in such a way as to promote their physical and mental well being.
  • Participates in meaningful and informative orientation of new staff nurse.
  • Follows and contributes to the development of OAS guidelines for providing services.
  • Provides psychiatric/mental health nursing care (as indicated) within the laws/licensure of the State of Michigan and to the full intent of Standards of American Nurses Association on Psychiatric Mental Health Nursing.
  • Maintains knowledge regarding patient rights and serves as advocate as needed.
  • Accepts and performs all other pertinent and reasonable assignments made by the Outreach Supervisor of Outreach Clinical Coordinator.
  • As indicated, provides physical health examinations of persons undergoing PASRR evaluation, and completes the PHYSICAL EXAMINATION, MEDICAL HISTORY, etc portion of documentation.
  • As indicated, makes follow-up visits to evaluated persons, primarily in nursing homes, to reassess current needs and evaluate nursing care.
  • Keeps Outreach Clinical Coordinator and Outreach Therapist advised of critical events and behaviors of evaluated persons, as indicated.
  • Attends professional conferences related to nursing care/management of elderly/mentally ill, as approved, and shares knowledge with other OAS staff, as well as nursing home staff (as appropriate), to improve the care to evaluated personnel.
  • Communication - contacts nursing home staff, families, physicians and NSO/Adult Services personnel regarding care and treatment of the PASRR evaluated residents.
  • Communicates with nursing home staff and physicians regarding physical health care needs of the PASRR consumer.
  • Communicates with relevant NSO-OAS staff regarding pertinent health care data and information exchange in relation to the PASRR consumer.
  • Contacts nursing homes, NSO staff, hospitals, families, and doctors for information regarding evaluated person's history and/or current status.
  • Accurately identifies residents' health education needs and provides for or recommends methods to meet those needs
  • Documentation includes the complete, accurate and timely submission of PASRR summaries, progress notes, nursing assessments, and other clinical and statistical records.
  • Records relevant information on initial and annual PASRR, Status Reviews, and other treatment reviews, as appropriate, and completes documentation within the specified time frames.
  • Appropriately records services on Progress Notes and elsewhere, as indicated, in clear and concise language and in the Unit format.
  • As indicated, completes nursing care documentation relevant to admission, transfer, and discharge of evaluated persons within program specified timeliness.
  • Reviews PASRR records, recommendations, and summaries.
  • Submits all documentation in a timely manner, consistent with program policies and procedures.
  • Summarizes labs, physical exams, current medication, consumer's needs, nursing care, medical diagnosis and treatment on PASRR record.
  • Performs any other job-related tasks as assigned by the supervisor.


Education, Licensure and Experience Requirements:

Education: Bachelor of Science Nursing (BSN) or Associates in Nursing (AN/ASN)

Required Licensure: Registered Nurse License (RN)

Minimum Required Experience: 5+ years' experience required if AS/ASN or 4 years' experience for BSN.

  • Knowledge of mental diagnoses and behaviors
  • Knowledge of developmental issues in the older adult population
  • Knowledge of how mental illness impacts older adults
  • Demonstrated competency serving older adults with mental illness.
  • Must have demonstrated computer proficiency, and be able to pick up new software as needed.
