Schedule requires some flexibility and may include evening hours, as needed.

Job Summary:
The Community Care Nurse at Huron Valley PACE works in a close collaborative relationship with a Nurse Practitioner, Registered Nurse and Licensed Social Worker to provide integrative care to frail elders with complex needs. The RN/LPN utilizes a systematic approach to practice incorporating critical thinking, planning, implementation and evaluation of outcomes to pro-actively manage and advocate for PACE participants. The focus of care provided is to enhance functional capacity, encourage autonomy, and assure coordination of care and services.

Primary Responsibilities:

• Provides wound care

• Provides medical care and oversight in coordination with the Interdisciplinary Team including, medication administration, participant and caregiver education, and service coordination

• Collaborates with the participant, family, and Interdisciplinary Team to develop the participant's plan of care

• Maximizes the participant's functional capacity by encouraging autonomy in all aspects of care

• Monitors participant progress with recommended treatments and notifies the appropriate team members of changes in participant status

• Recognizes the significance of abnormal assessment data and utilizes critical thinking skills when planning and implementing care

• Provides safe, comprehensive care to participants with complex health problems focusing on the individual participant and family

• Maintains accurate and timely records of participant functional and health status, progress toward care plan outcomes, revisions to care plans, care given, etc. All charting and documentation are performed in accordance with Huron Valley PACE policies and procedures.

• Advocates to others on behalf of the participant and demonstrates accountability in resolving participant concerns or issues

• Provides after hours on-call assistance via phone triage on a rotating basis

• Schedule requires flexibility in hours to coordinate care with participant family or caregivers after center hours

Skills and Knowledge:
Able to read, write, speak, and understand the English language.
Must possess the ability to communicate tactfully with participants, family members, visitors, team members and external agencies.
Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations
Must meet a standardized set of competencies before working independently

Required Qualifications:
High School Diploma or GED required
Minimum of one (1) year working with the frail, elderly or long-term care population
Licensed Practical Nurse or Registered Nurse with current Michigan licensure
Must possess a current State of Michigan driver's license and maintain an acceptable driving record
Must have reliable transportation available on a daily basis
BLS for healthcare providers or CPR Certification
