Job Summary
- Provides support in the telephonic response to patients' clinical needs and communicates to Providers to ensure ongoing clinical coordination and outcomes
General Accountabilities
- Completes and maintains accurate records or reports regarding patients' histories and progress, lab results, medications, services provided, or other required information.
- Provides telephonic case management and problem resolution for incoming patient and ancillary service calls
- Collaborates with health care providers both internal and external
- Manages and monitors diagnostic results and updates providers and patients with findings, tracks timeliness of results
- Reviews records and confers with other professionals to evaluate patients' conditions and to determine triage for Provider follow up and intervention.
- Coordinates with Providers patient treatment plans based on research, clinical experience, and client histories in conjunction to Infinity Visiting Physicians Policies and Procedures
- Reviews and evaluates clients' progress in relation to measurable goals described in treatment and care plans.
- Coordinates ancillary care services or other health professionals, such as doctors, nurses, social workers, home health, DME, pharmacy
- Update treatment plans and notes to comply with changes in client status, communicates changes and interventions with Providers
- Attends training sessions to increase knowledge and skills.
- Participates in case conferences or staff meetings.

Job Qualifications
- Education: Bachelor's degree in Nursing or a related field
- Experience: At least 5 years' experience in clinical nursing role
