Advanced Nurse Practitioner / Physician Assistant Role Scope
  • The APP works within an inpatient setting providing care to endocrinology patients
  • Assimilates the mission, goals and objectives of Hartford Healthcare systems into a structured approach that allows for the opportunity to provide excellent service, high quality care and fiscal integrity.
  • Provides vision and leadership for endocrine care with attention to continuous quality improvement and cost effective outcomes.
  • Serves as the professional resource and liaison to the hospital's central region's inpatient endocrine care program and initiates planned change in healthcare delivery as needed.
  • Coordinates the development and maintenance of inpatient diabetes protocols/care-paths/order sets.
  • Provides coaching, mentoring, peer review and guidance to health care providers in the provision of endocrine care.
  • Maintains personal proficiency of specialty competency and education skills in order to serve as an ongoing resource for team members, physicians and other staff.
  • Maintains familiarity with current national standards for recognition developed by the Joint Commission and evidence based practice developed by the American Diabetes Association and the American Association of Clinical Endocrinologists. Continually reviews program and processes to assure that all program sites comply with these requirements.
  • Actively participates in the work of the HHC Endocrine Collaborative Council.
  • Develops and implements provider education and in-services as needed.
  • Identifies opportunities to improve patient/customer experience. Participates in projects/programs designed to enhance service delivery and patient/customer satisfaction and ensure patient safety.

Qualifications

Advanced Practitioner Qualifications

  • Graduate of an accredited Nurse Practitioner / Physician Assistant Master's program preferred
  • Licensed, or eligible to be licensed, as an Advanced Practice Registered Nurse (APRN) / Physician Assistant in the state of Connecticut
  • Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
  • Board Certification as an acute care nurse practitioner required
  • 2+ years working within Endocrinology - preferred
