The Oncology Triage Nurse Navigator is a nurse who provides individualized care coordination to patients, families, and caregivers. This key function can help them overcome barriers within the healthcare system and facilitate access to all the resources available to all patients. The oncology triage nurse navigator acts as a link between patients and care providers to facilitate clinical and supportive care services. The nurse provides triage advice services to patients by utilizing the nursing process, conducting telephonic assessments in accordance with the protocol The role is that of a single point of contact among referring physicians, patients, and other clinical caregivers for the purpose of empowering patients in dealing with the disease and the treatment. The oncology triage nurse navigator facilitates patient care needs across the continuum of care.

Primary Responsibilities:
- Functions independently, utilizing the nursing process to triage patient telephone calls in accordance with approved protocols ensuring safe, quality patient care in the most cost effective setting.
- Serves as a patient advocate on the interprofessional team
- Transitions patients from one phase of care to the next, including self-management and independence in self-care.
- Answers or responds to calls from patients in a professional and timely manner.
- Assesses the caller's needs and determines the acuity and extent of the patient's condition, concerns, or progressive status.
- Assesses the caller to determine their understanding of the severity and extent of the patient's current condition and the caller's understanding and ability to carry out all instructions provided by the triage nurse.
- Matches unmet needs with referrals to a social worker, dietitian, genetic counselor, nurse practitioner, community or outreach coordinator, or applicable community services.

Required Qualifications:
- Graduate from an accredited program for professional nursing education, BSN preferred
- Current, unrestricted Texas RN License or current, unrestricted compact state RN License
- BLS certification
- One year of Oncology Nursing experience
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease.
- You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
- Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.
Preferred Qualifications:
- Oncology Certified Nurse (OCN) required (or willing to obtain within one year from start date)
- Oncology Telephone Triage experience
- Experience in management of cancer patients actively receiving chemotherapy or other infusion treatments
- Symptom management experience
- Expert knowledge in medical, surgical, and radiation oncology
- Knowledge of legal and professional guidelines and scope of practice
- Three or more years of Oncology Nursing Experience
