The Peer Navigator must have first-hand understanding of issues related to living with HIV or AIDS. As a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Medical Assistant responsible for helping people living with HIV (PLWH) navigate the healthcare system and the complexities of the diagnosis in a relatable, compassionate, and competent manner. The HOPE Peer Navigator - Registered Nurse (RN), Licensed Practical Nurse (LPN) or Medical Assistant is responsible for educating SLUHN and the community at large about HIV/STDs by utilizing nursing expertise. Nursing skills will also be applied in the clinic setting.
JOB DUTIES AND RESPONSIBILITIES:
  • Through one to one interactions, promotes patient engagement in healthcare and provides opportunities for education, skill-building, and emotional support in a respectful, non-judgmental setting. Serves as patient advocate and empowers/ coaches the patient to engage in self-advocacy.
  • Assists in the development of/coordinates peer mentorship program. Provides regular support and supervision to peer mentors.
  • Develops and maintains current HIV education curriculum to deliver to academic institutions and other community-based organizations.
  • Assumes personal responsibility to keep informed of changes and trends impacting the HIV community.
  • Develops and provides staff education on current & pertinent HIV-related issues on a quarterly basis to ensure staff competency.
  • Collaborates with the Linkage Coordinator and Patient Navigators to rapidly engage patients who are lost to care or newly diagnosed. This includes, but is not limited to, visiting patients in the hospital, home, and other medical appointments.
  • Conducts evidence-based prevention interventions, according to CDC and funding agency guidelines.
  • Provides individual or group counseling and education to patients when necessary.
  • Collaborates with Linkage Coordinator to act as a liaison between HOPE and the larger St. Luke's Network for HIV/STD related education and support.
  • Coordinates and participates in community outreach events, which includes HIV/STD testing, awareness, advocacy, and prevention education.
  • Functions as the primary Licensed Practical Nurse (LPN) or Medical Assistant during STD clinic.
  • Acts as back-up RN for infectious disease clinic or primary care.
  • Functions as visiting nurse when needed. Assesses patients and performs nursing interventions in accordance to the patient's plan of care and agency policy, procedures and standards. This includes, but is not limited to, medicine adherence, wound care, vitals, safety assessments, etc.

PHYSICAL AND SENSORY REQUIREMENTS:

Sit up to 7 hours per day; 3 hours at a time. Stand and/or walk for up to 6 hours per day. Frequently lifting, carrying and pushing objects up to 50 pounds. Occasionally lifting, carrying and pushing objects up to 100 pounds. Frequently stooping and bending. Frequently reaching above shoulder level. Must be able to turn, bend, climb, stretch, and lift in the provision of nursing care. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, high and low frequencies. Must be able to see as it relates to general near, far, color and peripheral vision. Must be able to speak as it relates to normal conversation and projection as it relates to lectures and presentations.

EDUCATION:

A graduate of an accredited school of professional nursing and possess a current active license to practice nursing in the Commonwealth of Pennsylvania. Or, as a Licensed Practical Nurse (LPN) or Medical Assistant.

TRAINING AND EXPERIENCE:

First-hand understanding of issues related to living with HIV or AIDS required. Two years as a professional nurse in nursing practice or previous community health experience as a professional nurse preferred. Or, as a Licensed Practical Nurse (LPN) or Medical Assistant. Must be trained or willing to be trained in phlebotomy and lab protocols.
