We are looking for a qualified Registered Nurse to join our nursing team on a temporary assignment. The Ideal candidate will have direct patient-care experience in a community health or primary care setting. RN candidate should have a desire to work with under-served communities and a passion for health education and primary care.

This position offers the ability to see patients both independently and as a vital team member in an integrated primary care model. Nurses in our clinics gain experience in case management, health coaching, triage, and furthermore are able to improve patient care through participation in and development of quality improvement initiatives.

40 hours per week schedule. Monday to Friday with flexibility to work evenings and weekends as needed.

Temporary assignment period: 6 Months

* This role requires a considerable amount of administrative nursing tasks, 60% administrative, 40% patient care.

1+ years of experience as a RN in Primary/Ambulatory Care highly desired.
Responsibilities:
Provides culturally-sensitive, patient-centered care to all MCC patients.

Provides nursing care within scope of license and clinic policy and procedures.

Conducts assessments and triaging of medical complaints for walk-in or late patients.

Performs telephone triage of patients' medical concerns and providing triage advice.

Reviews and follows up Emergency Department and Hospital discharge summaries to facilitate coordination of transition to community.

Conducts patient co-visits with clinicians to provide additional support in chronic illness management.

Utilizes health coaching skills and encourages patient activation and self-management.

Participates in intensive outpatient case management of medically or psychosocially complex patients in the clinics.

Participates in Quality Improvement projects and PDSA cycles.

Measures PT/INRs and assists providers in dose titration and monitoring of anticoagulant. medication.

Initiates referrals to public health programs and home-health programs.

Reports communicable diseases to county and coordinates follow-up.

Manages in-box messages from patients and medical providers

Administers injectable medications including insulin, antibiotics, and pain medication .

Administers routine immunizations per clinic standardized procedures.

Provides medication refills per clinic protocol.

Orders labs, medications, and vaccines per standardized procedures.

Reviews lab results, diagnostic imaging, and specialist consult summaries for actionable items.

Provides health education and formulating care plans for patients with chronic illness e.g., diabetes, asthma, heart failure, obesity.

Performs medication reconciliation as needed.

Performs nursing skills including insertion of straight or indwelling catheters, suture removal, wound care, and interpreting PPD skin tests.

Maintains and checks crash cart and AED.

Responds to medical emergencies in clinic and leads "MD STAT" situations alongside medical provider.

Provides support to Medical Assistants, including filling in as needed in the event of staff shortage or unanticipated need.

Coordinates pre-operative history and physical exams, including ordering labs or diagnostic studies per primary care provider.

Responds to in-person or electronic patient requests for lab results, medical questions and concerns regarding disease processes.

Qualifications:
Associate or BS Nursing degree required.

Active California RN License in good standing.

Current CPR Certification.

1+ years of experience as a RN in Primary/Ambulatory Care highly desired.

Excellent verbal and written communication ability in English.

Bilingual English/Spanish candidate is desired but not required.

Must be able to work independently and within a team environment.

Must be sensitive to ethnic and cultural differences and their relevance to health care.

Desired Additional Qualifications

Prior experience in a community clinic or primary care setting a plus.

Familiarity or proficiency in using NextGen Electronic Health Records or i2i Tracks.

Experience with nursing care coordination/case management.

ACLS or PALS certification a plus.
