Registered Nurse for Homecare Patients

Job Requirements:

• Providing nursing care to hospice or homecare patients
• Admit patients eligible for hospice/ homecare services
• Delivering high quality, compassionate hospice, oncology and palliative care for home hospice patients
• Provide hospice and palliative care at home
• Giving or assisting with personal care
• Provide direct patient care services
• Develop and implement home health plan of care
• Provide nursing services to patient/family
• Provide nursing care to patients
• Perform supervisory visits with the hospice team
• Coordinate the implementation of each hospice plan of care
• Complete documentation on hospice aide supervisory notes
• Provide home nursing care instructions to patients
• Evaluate patients for admissions to homecare
• Start of Care
• ROC
• Med visits
