Responsibilities:
Supportsorganizations mission by striving for excellence in all aspects of theirjob with a focus on positive interpersonal relationship with co-workersAssessesand evaluates the needs of patients with life threatening and lifelimiting illnesses and their families and develops an appropriate plan ofcareIdentifiesteaching needs and plans appropriate strategies for individuals andfamiliesMonitorsquality and appropriateness of services providedCollaborateswith patient and family, other members of the disciplinary team and otherhealth care providers in providing patient and family careDocumentsall changes in patients condition and updates care plans and outcomesAdheresto agency infection control and safety policiesAdheresto the organizations policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications:
