Communicate successfully with peers, supervisors, public, and staff members in a manner that represents Companysculture of a collaborative, positive work environment.
Supervise and coach the Care Services Nursing and Psychosocial staff. Conduct regular sessions and group meetings. Provide encouragement and guidance to staff.
Collaborate regarding orientation, evaluations and terminations. Provide consistent and proactive mentoring, coaching, teaching and counseling.
Provides effective leadership, decision making, and problem solving to ensure continuity of the referral/admissions process.
Facilitate communication and collaboration between Care Services Team and our internal and external customers.
Maintain and update knowledge of Companyprograms and the referral/admission process.
Communicate, collaborate and provide support to the Director of Care Services. Report weekly and as needed to the Director on the referral and admission processes.
Actively participate in quality management utilization review and risk management groups. Integrate quality into daily work in team activities and with team involvement.
In collaboration with the Director, assure adequate staffing through team scheduling and communication.
Ability to perform time management and organizational skills.
Utilize technology to facilitate the referral and admissions process. This includes computer software programs and telephonic systems.
Supervise the patient care activities of the team: coordinate and monitor the implementation of the care plan.
Monitor documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family plan of care.
Assure adequate staffing and staffing ratios through team scheduling and communication.
Assist in the efficient and cost effective management of team.
Provide leadership and formal and/or informal education to team, organization staff and the community through in-service presentations, team meetings, reports, case conferences or small group discussion.
REQUIREMENTS:
Nursing degree required BSN - MSN preferred. Prior leadership is a must. Hospice experience preferred.
