Overview:
St. Lukes Health system is currently seeking a Post-Acute Care Physician to join our highly collegial and patient focused, multidisciplinary Post-Acute Medical Group. The selected individual will help to oversee two skilled nursing facility (SNF) sites as the facilities Medical Director as well as provide direct patient care. This opportunity boasts a great work/life balance and call is only required one week every two-three months. St. Lukes providers partner with several skilled nursing facilities in the Treasure Valley and Magic Valley to provide a level of care that exceeds traditional past practices in nursing facilities and provides on-site availability for when care demands are highest. The St. Lukes Post-Acute Medical Group provides timely evaluations to both our short-stay patients and our long-term care residents needing chronic care and work diligently to prevent visits to the emergency department or hospital readmissions.
