The Sans Souci Rehabilitation and Nursing Center is looking for a talented and hard-working LPN (Licensed Practical Nurse) . to join our ever-growing team.
Sans Souci Rehabilitation and Nursing Center is a facility with 120 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 40 + short term beds. Our Sans Souci Rehabilitation and Nursing Center team consists of 20+ Therapists spanning 20 years of experience with backgrounds in acute, sub-acute, long term and outpatient settings would provide an excellent setting for learning and growth. Sans Souci Rehabilitation and Nursing Center provides Rehabilitation services 7 days per week.
Responsibilities Include:
The LPN responsibilities include but are not limited to:

Monitor, plan and implements the nursing care of residents within the facility under direction of a RN.
Ensures continuity of care of residents between shifts by providing direct care as well as supervising the care given by C.N.A.s and support staff members.
Participates in resident and family teaching.
Serves as resident advocate with focus on the facilitys mission.

Qualifications/Requirements:

Current/valid NYS LPN license/registration
1-2 years current experience in long term care

Shift -

Per Diem
Day Shift (73) Weekends
