About the position:

Field Registered Nurse provides ongoing coordination of care in the home setting. The Field Nurse visits clients at home and continually evaluates the clients needs in the home setting. The Registered Nurse coordinates the client's needs and follows the individualized treatment plan developed with the primary care provider.

Qualifications:

• Excellent organization and communication skills.
• Excellent clinical assessment skills
• Strong ability to collaborate with Team Members and work independently
• Current NYS RN license and registration
• Prior Homecare or Visiting RN experience
