Requirements:

• School Diploma or Completion (RN/LPN/CNA)

• NYC State License and Registration

• Driver's License or State ID and SSN

• Resume and 2 Reference Letters

• Physical Exam/PPD/ MMRV

• Flu shot vaccine

• CPR or BLS (Optional for CNA)
