The primary role of the nurse coordinator is to ensure seamless coordination of patient care across the continuum. This includes clinical practice, care coordination, education, quality improvement, with significant latitude for exercising professional judgement. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families within all Pediatric Treatment Centers. The Nurse Coordinator acts as liaison between patient, family, provider, clinical staff, pharmacy, and scheduling and registration personnel to ensure safe and efficient patient flow.EEO/AA/Disability/Veteran

Responsibilities:
1.Professional Practice:1.1The Pediatric Nurse Coordinator is the lead member of the health care team coordinating all aspects of patient care for the specific specialty.2.Education2.1Provides patient and family education on appropriate medical condition, including signs, symptoms , treatment regimens, side effects, outcomes expected, and potential adverse reactions or outcomes. Including manangement of condition, promoting optimal level of health.3.Multi-Disciplinary Collaboration3.1Functions as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum4.Quality Improvement4.1Participates in data collection related to clinical and operational efficiencies.

Qualifications:
EDUCATIONRegistered Nurse , Bachelor's Degree in Nursing preferred, Connecticut RN license.EXPERIENCEAt least 2-3 years experience in pediatric hematology/oncology nursing and direct chemotherapy/biotherapy administration. Experience in ambulatory setting preferred.LICENSURELicensed as a Registered Nurse from the State of Connecticut. APHON certification.SPECIAL SKILLSExcellent interpersonal and organizational skills. Ability to assert oneself, advocate for patients, and work independently with minimal supervision. Ability to manage multiple priorities. Computer experience and knowledge of word processing and data entry systems. Must be able to communicate efffectively with all members of the healthcare team, patients and families. Must value and execute "patient -centered model of care."PHYSICAL DEMANDMust be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs

Additional Information:
Pediatric Hematology/Oncology experience required with chemotherapy/biotherapy administration certification. Excellent interpersonal skills to help coordinate patient care. Ability to advocate, communicate and help navigate patients and families through the continuum of care. Ability to manage multiple priorities and communicate effectively with all members of the healthcare team.
