Halifax Healthis seeking an Advanced Practice Registered Nurse (APRN) for our Wound Care Department.
Summary:
The Nurse Practitioner is an independent health care provider who, working collaboratively within a multi-disciplinary health team, is responsible for patient evaluation and care through the performance of pertinent history and physical examination, assessment of needs and delivery of appropriate treatment. Provides nursing care and health teaching to an assigned group of adult and geriatric patients in an observation setting. All duties andresponsibilities are accomplished according to the collaborative practice agreement required by the State of Florida.
Job Qualifications:
Graduate of an accredited program in nursing, MS in nursing is requiredLicensed by the State of FloridaWound Care Experience required, Certification in Wound Care preferredHyperbaric Oxygen experience preferred; or 40 hour approved training completed within the first month of employment.Professional knowledge of nursing theory and practiceExtensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and management of acute chronic patient problems
Job Duties and Responsibilities:
Provision of hands-on care to patients as directed by Halifax Health Wound Care Physicians.Debridement of wounds by sharp, enzymatic or autolytic methods.Application of topical wound care and associated wound management products.Recommendation of associated services to improve wound healing outcomes. This includes, but is not limited to, labs, radiologic tests, vascular tests, referrals, home health care, Negative Pressure Wound Therapy device, and orthotics.Interpretation of diagnostic tests and recommendation of appropriate follow up to same.Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines.Patient education and management.Communicates in a clear and understandable manner using appropriate verbal and non-verbal styles.Actively participates in departmental activities to improve education and patient quality metrics.Assists with one-on-one training and coaching sessions as assigned.Attends required meetings and participates as a committee member as requested.Assists with special projects as needed.Maintains strict confidentiality
