Home Care Licensed Practical Nurse (LPN)

Summit Home Care New York - Summit Home Care is a family-owned and operated home care agency owned by a Registered Nurse. We service all counties of NY and are currently seeking exceptional LPNs to join our team. We are a full-service PDN agency that services both the pediatric and geriatric populations. We have an immediate need for LPNs to fill shifts; nights, days, and weekend hours.
As we look for experience with GT, Trach & Vents we do have a phenomenal education program offering training to new grads. This opportunity awaits motivated and ambitious professionals looking to work under the direction of our Nurse-Managers.
Responsibilities:

Follow a plan of care designed by our Nurse Manager
Perform Medication Administration and Treatments per POC
Monitor clients' conditions and report accordingly
Accurate documentation in accordance with the provided orders

Requirements:

Valid NYS License
Physical with PPD or Quantiferon (cannot be older than 6 months)
If the PPD is positive then a chest x-ray is required
Drug screen test (cannot be older than 6 months)
Rubella & Rubeola blood work (from the lab)
Two (2) typed references
Social security card
Valid ID (NY State ID / Passport / Resident Card)

Experience:

Nursing: 1 year (Required)

License/Certification:

BLS Certification (Required)
LPN RN (Required)
