Job Position

Rendr 360 Department: Physician Assistant

Rendr 360 is a department of Rendr which provides centralized support to our medical practices: care management, care coordination, continuous quality improvement, etc. We are a growing division with many patient-centered projects which provide continuity of care for patients between office visits to help improve clinical outcomes and patient satisfaction.

Essential Functions:

  • Conduct house call and virtual visits on behalf of the Rendr enterprise.
  • Evaluate, diagnosis, treat and manage patients following current evidence based guidelines and best practices.
  • Collaborate with the Rendr 360 team to manage a caseload of patients with moderate- to high-complexity medical issues referred by other staff in the medical network using care coordination to promote quality, cost-effective care
  • Follow-up with patients between primary care and specialty visits, including review of associated lab and radiology reports and specialist recommendations, including pre-authorization of tests and referrals
  • Develop, assess, and adjust, as necessary, the care plans of patients based upon assessment with specific objectives and interventions to promote desired outcomes
  • Assess the patient's current health status, resource utilization, past and present treatment plan and services, prognosis, short- and long-term goals, treatment and provider options
  • Provide patient/family and staff education
  • Facilitate patients' access to network and community-based services
  • Participate in all Chronic Care Management activities of patients enrolled in this program in accordance with Centers for Medicare and Medicaid guidelines
  • Participate in patient care quality programs such as Transitional Care Management, telemedicine, population health, and continuous quality improvement projects
  • Participate in departmental, interdisciplinary, and network committees and meetings as appropriate
  • In collaboration with supervising physician, may provide direct patient care through perform physical examinations and patient assessments to manage a range of patient health and wellness issues
  • Other special duties and projects as needed

Qualifications

  • Active NYS PA license required
  • BLS/ACLS certification required
  • Excellent interpersonal, teambuilding, and communication skills
  • Demonstrated compassionate and caring nature
  • Dedicated to providing high quality healthcare and patient experience
  • Exemplifies patient focused customer service skills with ability to build rapport with patients
  • Previous clinical experience in an ambulatory care setting preferred
  • Bilingual English and Chinese required (Mandarin or Cantonese)
