Responsibilities:
Great New Career Opportunity in Palliative Care!Now Hiring - Palliative Care Medical Doctor- Remote work opportunityThe Palliative Care MDwill be responsible, but is not limited to:Leads Palliative Care Interdisciplinary Team Meetings, reviews admissions, develops patient care plans to manage and alleviate symptoms, reduce ER visits and rehospitalizations.Insures that appropriate patients are admitted to Grace Health Palliative Care Service.Facilitates patient transition to hospice when clinically and medically appropriate. Completes MD admission documentation for each patient timely.Reviews patient medications and recommends medications at time of admission and at any time point while the patient is on service, including at IDG.Engages patient and family in advance directive planning discussions via phone, virtual meetings;Recognizes and performs duties which need to be performed although not directly assigned.Completes all required orientation and annual learning requirements on time.Educates staff on current palliative care clinical management and interventions, disease processes, adjunct therapies.

Qualifications:
Education/Licensure:Board Certified in Palliative CareActive MD license to practice in state of residence Professional Experience: Bi-Lingual2-4 years Palliative Care and or hospice experiencePhysican trainingMust be willing to travel
