*Starting rate $70,000The Registered Nurse (RN) participates in the coordination of medical, behavioral health/addiction treatment services and other supports in accordance with the person-centered treatment plan, and works collaboratively with the BHCP team at their respective agency to manage or co-manage complex behavioral health cases. Responsibilities:
In collaboration with other CCBHC nursing and medical staff, provide primary care screening; monitor health indicators such as blood pressure, body mass index, lipids, and glucose; monitor lab work, adverse effects of medications, and use of tobacco products.Provide teaching, consultation, and on-site support to CCBHC staff by assisting in the development of strategies to identify and address the physical health needs of CCBHC participants.Consult with care coordinators and other staff to support their involvement in facilitating transitions in care and attending hospital discharge meetings and post hospitalization follow up visits.Participate in CCBHC Wellness Team and other Integrated Treatment Teams.Participate in training and supervision on the CCBHCs identified Evidence-Based Practices (EBPs) and provide treatment consistent with these practices, particularly Screen, Test, Immunize, Reduce Risk, Refer and Smoke- Free Program.Engage individuals in participating in other CCBHC services, E.g. Peer Support, Recovery Coaching, Care Management, Psychiatric Rehabilitation Services, Social Support Opportunities, and Community Recovery Supports.Actively participate in the Quality Improvement process of the CCBHC.Maintain all records in accordance with the policies and procedures of the CCBHC.Practice in general accordance with the treatment philosophy of the CCBHC.Maintain open, collaborative, and collegial relationships with internal and external team members.Attend trainings as assigned; maintain necessary certifications required by Advocates.Maintain valid CNS/NP certifications.

Qualifications:
Registered Nurse licensed in the Commonwealth of Massachusetts with minimum of 4 years of experience working with adults in a community-based mental health, substance use and/or physical health settingDemonstrated knowledge base and judgment necessary for independent clinical decision making.Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required. Excellent organizational, time management and problem solving skills.Strong computer knowledge, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.Knowledge regarding psychiatric rehab and understanding of recovery model.Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.Must be able to perform each essential duty satisfactorily.Must hold a valid drivers license.
