North Chandler Place is currently hiring LPN's for open FT/PT Night (12 hour) shifts!

As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and North Chandler Place standards. Your role will involve monitoring and reporting changes in the patient's condition on a daily basis, you must be able to assign duties, instruct, and provide clinical supervision to CNAs.

RESPONSIBILITIES & ATTRIBUTES:
  • Record physical findings, and formulate plans and prognosis based on resident's condition. Report these to the attending physician.
  • Give direct physical and psychological nursing care in emergencies, as well as in the presence of an illness or disability, in order to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope.
  • Provide information to resident and family members of prevention techniques and other ways to maintain or improve the general health and psychosocial status of the resident.
  • Must be able to deliver quality nursing care knowledgeably and competently to patients
  • Be a Force on the Floor: Lead team by example when passing medications, assisting residents with personal care, and completing paperwork.
  • Communicate effectively to RN Supervisor all residents change in conditions

GENERAL REQUIREMENTS
  • Valid and active state-issued Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Valid AZ Level One Fingerprint Clearance Card or proof of Fingerprint card being processed
  • Current first aid/CPR certification (BLS)
  • Experience as a Licensed Practical Nurse (LPN) preferred, but new grads welcome.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • High School Diploma or equivalent
