The Nurse coordinates interventions and facilitates treatment plans for consumers admitted to the crisis facility. Able to collaborate with prescribers in all phases of medication management and follow-up, medication groups, medication problems and side effects Identify potential mental health emergencies and necessary interventions. Identify potential physical problems impacting the mental health of consumers.
The working hours for this position are Mon-Thurs 8 pm - 6 am.
Responsibilities:
Essential Duties: Appropriate medication dispersal
Coordinate with pharmacy about consumer admissions and discharges to provide continuity of care in dispensing medication to appropriate site
Monitor consumer medication for needed refills, medication changes and new medication orders every shift
Review electronic chart to ensure all medication changes are reflected in separate paper chart
Review medication administration record for each consumer each shift to review medication given as ordered and PRN medication given during previous shift
Review medication on hand to determine instruction labels are correct per current medication orders for each consumer
Monitor the recording and count of controlled medications each shift
Monitor medication dispensed in pouches to ensure any medication provider discontinued is removed from pouch and inform pharmacy of change
Appropriate documentation to include electronic chart and separate paper chart required by health department
Provide on-site medical screening as needed
Provide medical screening of consumers referred for admission to determine conditions requiring specialized care needs and/or specific medical equipment
Coordinate with referring liaisons/clinics regarding medical conditions requiring special care needs and/or equipment being available prior to admission of consumer
Coordinate with referring liaisons/clinics consumer medication and current orders are available upon consumer admission
Consult with center on call medical providers during off hours when necessary for appropriate provision of consumer care
Coordinate with center and community residential facilities to provide necessary documentation required by health department regulations prior to consumer transfer to the designated residential site
Help monitor milieu.
Assist in medication groups, as required.
Able to set up monitored medications through the use of medication boxes.
Coordinate with outpatient clinics for transfer of medications not placed in medication boxes upon discharge of client from facility
Provide consumer date for medication refill at outpatient site upon discharge
Perform injections.
Review and order labs, notify prescriber of any abnormal labs requiring immediate attention.
Monitors vital signs and document nursing functions.
Assist prescribers in tracking and meeting timeframes for annual labs and AIMs
Ensure compliance with OSHA regulations.
Ensure compliance with health department regulations
Coordinate medication processing and applications with PAP office. (since LOS is relatively short period of time this may not be applicable)
Provide consumers with medication education.
Monitor consumers for side effects of meds, including AIMS testing or any medical condition that may affect their mental illness.
Provide group treatment as determined by the Network Director.
Engage consumer and family in the treatment process and goal-setting. Identify risk factors including lethality. Work constructively with consumer to reach agreed upon outcome, and coordinate care with internal and external providers.
Monitor and review consumers' charts for medication/medical issues with appropriate follow-up.
Coordination of care with consumers' prescribers, including documentation of medical medications & dosing, physical diagnoses, monitoring of potential medication interactions with psychiatric medications, and referrals as indicated.
Submit prior authorizations to consumers' insurance in a timely fashion; address medication changes with prescribers as needed.
Coordinate with consumer treatment team for continuity of care upon consumer discharge from facility
Other Duties:Attend mandatory in-services, compliance with individualized training plan if required.
Participate in supervision by coming prepared with an agenda. Report high risk/problem cases, and utilize a problem solving approach as well as feedback. Attend supervision at times and intervals agreed upon with supervisor.
Provide group treatment as determined by Network Director.
Engage adult consumers and their families in the treatment process and goal-setting, primarily for medications only clients. Identify risk factors including lethality. Work constructively with consumer to reach agreed upon outcomes, and coordinate care with internal and external providers.

Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public.

Maintain consumer confidentiality per HIPPA regulations
Volunteer to work on Center committees and/or task forces.

Qualifications:
Education, Knowledge, Skills & Experience Required:LPN with addition of two or more years of experience and/or training in a psychiatric setting.
Candidate must have knowledge of psychiatric conditions and symptom manifestations
Candidate must have a good understanding of psychotropic medications and their side effects.
Candidate must have developed awareness of various medical conditions, medication interactions or side effects that may affect the health or psychological functioning of a client.
Applicant must have basic computer skills such as e-mail and prescription writing.
Salary & Benefits:
Grade 12-Licensed Practical Nurse
$44,100 to $58,800
*Salary is determined based on years of total relevant experience.
*Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.
BONUS OPPORTUNITY!! Three phase bonus paid out using the following schedule. Based on full time (40 hours/week). Prorate if less than 40 hours/week. Must work a minimum of 20 hours/week to be eligible for bonus.
