Come and join an Award winning team of hospice and home health professionals What you will do Work flexible schedule and hours close to home work as much or as little as you want Assist patient with hands-on personal care: hygiene and grooming tasks, elimination, nutritional support, ambulation and exercise, identification and reporting of patient changes including skin conditions according to the patients needs and as specified on the Patient Assignment/Plan of Care. Ensure quality and safe delivery of CNA services per written plan of care. Communicate with RN, Rehab Case Manager or Clinical Supervisor on a regular basis regarding changes in the patients condition and/or needs. Perform other related supportive tasks as specified by Agency policy. Additional list of duties and responsibilities is provided in the Job Description Who you are: Completion of an approved training program pursuant to Federal/State law [R9-10-11-3(D)] [484.80] Current Arizona State Board of Nursing, Nurse Aide Register certificate and fingerprinting. Current CPR and TB and health clearance. Successful passage (80%) of Agency competency exam and core competency checklist. DMV and criminal background check / fingerprint card as appropriate. Have the orientation and training appropriate to the care of the patient and his/her family.
