Providence is calling a Clinic RN - General Cardiology Gateway (1.0 FTE, Days) to the Oregon Clinic Building in NE Portland, OR.

We are seeking a Registered Nurse (RN) Clinic - Heart Clinic Gateway that will provide comprehensive nursing care for patients in an ambulatory care environment based on individual physical, emotional, and spiritual needs, and appropriate care strategies throughout the lifespan. This position facilitate the workflow in the area by conducting nursing assessments and documentation. In addition, the Clinic RN contribute expertise in the areas of patient scheduling, telephone advice, and pharmacy needs. The Clinic RN assist with maintaining compliance to professional nursing standards and regulatory requirements and supports physicians in delivering quality health care to patients.

In this position, you will have the following responsibilities:
  • Provide or assist in assuring direct patient care in person, telephonically, or electronically according to clinical best practice standards
  • Collaborate with interdisciplinary team members to provide comprehensive care for patients and families, may provide case management and care coordination services for patients, as needed
  • Interface with various PHS departments as needed to facilitate patient care (home health, ED, lab, infusion, DME, or other departments)
  • Participate in the development and/or implementation of evidence-based quality improvement strategies
  • Assist with monitoring for compliance, quality assurance, utilization management, preventative guidelines, and accreditation requirements
  • Provide educational and care coordination for patient populations by teaching, supervising and counseling the patient and caregivers regarding nursing care and other related problems
  • Provide education regarding medication, symptom management, health teaching, and anticipatory guidance to patients and their families

Qualifications:

Required qualifications for this position includes:
  • Associate's Degree in Nursing
  • Current unencumbered Oregon RN license
  • Current American Heart Association (AHA) - Basic Life Support (BLS) certification
  • Advanced Cardiac Life Support (ACLS) certification within three (3) months of hire
  • One (1) year of Nursing experience

Preferred qualification for this position include:
  • Bachelor's Degree in Nursing -OR- related field
