Hospice Liaison (Nurse or Social Worker Liaison) $5,000 Sign on bonus

If you join our team your territory will be a focused area of King County. Although you'll be in the field the majority of your time, our offices are located in Georgetown where we hold training and meetings and opportunities to meet the team!
Kline Galland Hospice is a member of Kline Galland's continuum of care; Seattle's Single Sources Senior Care Resource serving Seattle for over 105 years!
We are immediately hiring for a Hospice Liaison and includes a $5,000 sign on bonus.
Your key responsibilities will be

* Bringing Kline Galland Hospice services to patients, family members, PCPs, other physicians and practitioners, ALF, AFH, SNF and other referral sources
* Working with referral sources to make the admission and discharge planning more seamless
* Consistently advocating for the referral sources and patients utilizing Kline Galland Hospice and reporting back feedback to operations team
* Assisting referral sources and patients by informing them of other Kline Galland service lines and facilitating the process to utilize our services
* Demonstrating the ability to meet productivity goals

If you desire to join our team you will need:

* Current WA State LPN or RN license or a Medical Social Worker
* Valid driver's license, auto insurance and dependable vehicles
* Prior experience in in a multiple service health care model preferred including nursing homes, hospitals, hospice and home care.
* Prior experience in Hospice is a huge plus but we're more interested in a resourceful LPN/RN or Social Worker with business development background that has a passion and interest in Hospice.
* A proven track record with business development
* Referral relationship management
* Experience in long term care admissions and marketing, case management, and/or hospital discharge planning.
