The Wound Care Nurse will identify patient and staff educational needs, facilitate the development of educational programs, and ensure smooth day-to-day operations.
The Wound\Ostomy Care Nurse is a weekday, Monday - Friday position with no evening, or on-call requirements. Weekend hours will be very occasionally required.

RESPONSIBILITIES:
- Works collaboratively with Clinical Team Manager to select wound core team members and to ensure smooth day-to-day operations within respective team, ensure the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resources utilization.
- Provides leadership to visiting staff to ensure appropriate multidisciplinary communication, collaborative working relationships, and a customer-oriented environment responsive to the needs of external/internal customers. Is easily available to assigned visiting staff for consultation, co-visits and problem solving. Provides input for Wound Core Team members annual performance evaluations.
- Coordinates and manages the use of and review of the McKesson Wound Advisor program. Monitors new WOCN referrals and consultations of new or changing wounds through the electronic functionality. Provides agency wound data and outcomes based on Wound Advisor information. Supports staff in the use of the functionality.
- Participates in orientation of new staff, acting as a resource, supporting the preceptors, and identifying opportunities for staff development for new oriented as well as all team members.
- Works collaboratively with the manager to facilitate the provision of quality care and case management through: home visits, co-visiting, case conferencing, chart audits, and financial analysis.
- Is responsible to establish short and long-term objectives, identifying areas of improvement for promoting both quality and cost effective patient care, and team/service evaluation.
- Performs co-visits with PHV as needed to support ongoing competency of staff and provides CTM's with feedback of staff competence via co-visit documentation form.
- Conducts ongoing WOC program development, collaborating with the Director, Quality Improvement, and partners throughout Strong Health. Become a member of the national WOCN society & attend WOCN affiliate and regional/national conferences to continue development of the WOC program.
- Works collaboratively with the Supply Management Department in relation to annual review of the wound formulary and with wound ostomy supply vendors, aiming for quality cost effective outcomes. Day to day problem solving with the Supply Management Department with regards to wound, ostomy, continence supply issues.
- Approves any wound/ostomy supplies outside of the standard wound formulary.
- Assists in identifying new marketing opportunities, conducts community education and marketing on an ongoing basis.
- Works corroboratively and forms professional relationships with various supply/distribution vendors and local DME providers, remain up to date on newest dressings/products in order to give patients the best care possible.
- Mentors and encourages current and future WCC staff to enrich the expertise throughout the clinical operations to benefit patient outcomes.
- Responsible for the Key Performance Indicators (KPI's) established by their manager.
QUALIFICATIONS:
- NYS licensed RN, BSN, Master's Degree preferred, and WOCN Certification and experience required.
- Community or home health experience preferred.
- Expert in interviewing, assessment, communication, and case management skills, with current and comprehensive knowledge of community health nursing preferred.
- Ability to mentor professional staff in case management of a patient caseload, including complex and high-risk populations.
- Demonstrates good interpersonal and communication skills, both oral and written.
- Demonstrates excellent teaching and mentoring skills.
