We are growing and actively searching for providers in DuPage County and surrounding areas to join our team on a PRN basis.

POSITION DESCRIPTION

As a Nurse Practitioner for IHA you will be responsible for performing Health Risk Assessments and possibly transitional care for patients discharged from hospitals to prevent readmissions.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Conducts comprehensive in-home health risk assessments to identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit.
  • Educates and counsel patient and family on any conditions identified during assessment and screening procedures.
  • Makes recommendations and communicate findings to Primary Care Physicians through documentation of outcomes.
  • Performs follow up visits in transitional care

REQUIRED KNOWLEDGE, SKILLS, AND EXPERIENCE
  • Current RN and NP licensure in the state of practice
  • Current CPR certification
  • Must be enrolled in both Medicare & Medicaid
  • Minimum 1 year of clinical experience as a Nurse Practitioner in Hospice or Palliative setting
  • Excellent communicator
