Heart to Heart Home Care - LPN - Full Time and Part Time (Group Home)
Heart to Heart Home Care is seeking professional, compassionate LPNs who are enthusiastic about making a positive difference in the lives of our consumers.
JOB DESCRIPTION:
Group Home Licensed Practical Nurses provide assistance with medical needs, personal hygiene, medication administration in the Group Home setting. There are 3 to 4 consumers in each of our residential Group Homes that primarily need the skills of LPN for the following: PEG tube, Seizure Management and Diabetic Care. Group Home schedule is primarily 7am-3pm, 3pm-11pm and 11pm-7am (9am).
JOB DUTIES INCLUDE:
Provides Nursing services to patients under the direction of a Registered Nurse.
Identifies and reports to the coordinating nurse, health needs of patient(s) through the observation of their physical, social and emotional needs.
Administers nursing care to patients according to the care plan.
Records and reports observations and nursing care performed.
Performs other related functions as requested by the supervising nurse within the scope of practice and related experience.
Administering and supervising medication.
Maintaining proper documentation.
MINIMUM REQUIREMENTS:
Complete Physical within the year
Pass a Criminal Background Check/Pre-employment drug screening
1 step PPD
LPN License
Driver's License & Social Security Card or Birth Certificate
CPR/ First Aid Certification (If not certified, training will be provided)
2 References (1 professional/1 personal)
