REQUIRED QUALIFICATIONS:
1. Two quarters of college or Nursing Assistant certificate or three months of previous work experience in a health care setting involving patients. Must be able to lift a minimum of 30 pounds.
2. Aptitude and interest in working with both medical and psychiatry patients in a primary care setting as part of a team.
3. Demonstrated proficiency in working with computerized medical records, Microsoft Word, and other software.
4. Demonstrated attention to detail and persistence in completing tasks.
5. Demonstrated ability to multi-task, organize tasks and prioritize.
6. Demonstrated high-level communications skills to work with patients, staff, providers, and outside agencies.
7. Demonstrated excellent customer service skills.
8. Respects and works with patients and staff from diverse cultures, languages, and backgrounds.
9. Capacity to work on a team and make positive contributions to team efforts.
10. Must be CPR certified within 60 days of start date and must continuously maintain CPR certification throughout employment.

PREFERRED QUALIFICATIONS:
1. Fluency in Somali (preferred), Spanish or Hmong or Vietnamese or Laotian.
2. Experience working in a Community Health Center or with clientele who experience many barriers to health care.

About the Job

POSITION SUMMARY
CUHCC's mission is to transform care and education to advance health equity. This position will work alongside the providers, interpreters, nurses and other members of the patient's care team to provide for patient visits, room patients, provide testing, vaccinations, and other services, help complete forms, etc. Nursing Assistants also have other roles in ensuring patient flow, cleaning and stocking exam rooms, and managing the clinic visit support functions.

ESSENTIAL FUNCTIONS:
1. Patient Contact/Care: 85%
1. Conducts thorough and timely pre-visit planning for each encounter according to standards.
2. Prepares for each visit according to the pre-visit plan.
3. Assists with psychiatric or medical needs as they arise: performs EKGs, makes same day referrals to medical, lab follow up and hospital transfers, or other.
4. Interviews patients, checks vital signs, performs assessments appropriate to patient age and visit type, obtains consent, prints out lab reports, hospital discharge summaries, and related documents.
5. Coordinates with scribes and interpreters for each patient visit.
6. Completes providers' orders as appropriate.
7. Assists providers with procedures and exams.
8. Administers medications and injections, including vaccines.
9. Teams with other medical/nursing assistants, laboratory and nursing staff, PSR's and others to support patient care and flow.
10. Contacts patients with lab results or other needs.
11. Provides patient health education materials around diet, symptoms or other areas as outlined in the after visit summary and/or care plan as directed by a provider.
12. Coordinates with the patient and other members of the care team to ensure patients receive assistance for medications, find transportation, or connect to community resources (food shelves, groups, shelters) as needed.

Other duties: 10%

• Maintains examining rooms/replenishes supplies/cleans instruments.

• Inventories and orders supplies.

• Actively participates in daily huddles, team meetings and other clinic initiatives.

• Participates in quality improvement projects to improve clinic flow and improve patient outcomes.

2. Other duties as assigned. (5%)

CUHCC reserves the right to modify this job description within the parameters of the job classification. This position requires a negative Mantoux test and proof of completion of the Hepatitis B vaccination series. The vaccination series may be completed within 6 months of initial employment.

Salary Range: $16.24 to $19.20 USD/hour
