Overview:
LifeLong Medical Care has an exciting opportunity for a part-time (20 hours a week) Family or Pediatric Nurse Provider or Physician Assistant (FNP/PNP/PA) to join our school-based team - Elmhurst United Middle School in Oakland, CA. There is an opportunity for full-time hours if splitting with another site within LifeLong is desirable. The ideal candidate must commit to providing high-quality primary care for complex patients, be dedicated to building a collaborative team model of care, and ideally experience working with Covid-19 related care. Communication is a vital component of a triage provider's job. In addition to taking critical action and talking with patients about their condition, triage providers relay patients' conditions to other care team members, including other providers and nurses, and provide patient information. We provide medical, dental, behavioral health, and school-based services at 16 primary care sites across Oakland, Berkeley, and West Contra Costa County. From opening a residential respite program for homeless persons discharged from hospitals to offering free COVID testing to the community, we are innovative and responsive to our times' social and medical needs. Benefits:We offer a competitive annual salary of $123,000-144,000 for NP/PA and $197,000-$218,000 for MD/DO depend on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs, and commuter benefits. LifeLong is qualified for federal and state loan repayment programs (HPSA score is 21). Acceptance rates may vary year to year based on funding levels and the number of applicants. Responsibilities:
Ability to make quick decisions, multitask and handle stress; strong written and verbal communication skills; ability to work as part of a team; interpersonal skills; compassion.Participate in the coordinated care for patients seeking care via telephone and MyChart messages Follow up with patients and ensure communication with the primary care team.Coordinates with Care Transitions team for all hospitalized patients to ensure timely follow-up and return to primary care.Uses clinician-approved standardized procedures to order labs and preventive screenings and to refill routine medications.Coordinates care closely with other team members to identify patient self-care goals and ensure that medical and psychosocial needs are met.Acts as a resource for support staff. Facilitates follow-up care as needed. 1. Provides preventive, urgent care, and primary care health services Performs clinical assessment of patients, including ordering appropriate diagnostic evaluations, and plans a program of care. Provides verbal and written education and counseling to patients, families, and caregiversPlans follow-up care, including requests for outside consultations and evaluations.Maintains client records necessary for clinic reporting, billing, and quality improvement.Consults with the supervising physician as appropriate 2. Supports and or participates in triage functions, both by phone and walk-inRecognizes and evaluates situations that call for the immediate attention of the physicianProvides interim or definitive care 3. Functions potentially in a variety of settings including clinic, skilled nursing facilities, supportive housing sites, and in clients' homes 4. Participates as a member of the site clinical services teamAttends clinical team meetingsProvides specialty care services, clinical case management, and team participation to designated programs (e.g., HIV services, women's health, house calls, diabetes, Bridge, etc.)Coordinates care with other health team members, including case management and behavioral health staff.Acts as an advocate for the patient and liaison with outside agencies as appropriateParticipates in quality performance activities and improvement initiatives 5. Participates as an active member of LifeLong Clinicians Association and abides by its Code of Conduct. 6. Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives according to the organization's mission and strategic goals, federal and state laws and regulations, and accreditation standards. 7. Other duties as assigned.

Job Requirements Current California Nurse Practitioner and or Physician Assistant's license.Experience in the delivery of health care in a Family Practice setting preferredCommunity health clinic background preferredProficient in Microsoft office suite Electronic health records, EPIC preferred.Bilingual in Spanish is required
