  • Purpose of Job
    • Accountable for the delivery of quality care to patients and families according to the individualized needs of the patient.
    • Assist the RN in the admission and discharge of the patient.

      • Graduate of accredited school of nursing required.

      • Current valid Licensed Practical Nurse (LPN) License, valid compact multistate license to work in Iowa or temporary permit while awaiting licensure required.
      • Current American Heart Association Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.

      • Assist the Registered Nurse (RN) by obtaining initial assessment of patient and on-going patient observation.
        • Completes initial assessment and documentation within standards set by nursing service.
        • Informs team leader/director of change in patient's status immediately.
        • Utilizes policies and procedures and Standards of Care to assess patient.
      • Performs nursing care as assigned by Charge Nurse, utilizing policies and procedures, Standards of Care and Care Plan.
        • Demonstrates skill in conceptual, technical, and procedural nursing practice to include unit specific policies and procedures outlines from orientation checklist.
        • Performs patient care and treatments in a timely manner.
        • Documents care and patient responses following policies and procedures for documentation.
        • Safely and accurately administers medication without incident.
        • Reports variances to team leader/director immediately.
      • Conducts patient teaching according to needs identified by discharge planning and care plan.
        • Conducts patient education utilizing available resources and documentation on patient education form.
        • Documents appropriate information on discharge forms.
      • Provides safety measures to protect staff and patients.
        • Utilizes patient restraints and documentation according to hospital policy.
        • Utilizes bed rails according to policy.
        • Reports safety hazards to director/charge nurse immediately.
        • Completes variance report within eight (8) hours of occurrence or incident.
        • Removes unsafe or unnecessary equipment.
        • Keeps room in a neat and orderly fashion.
      • Communicates verbally and in writing with patients, visitors, employees and medical staff.
        • Demonstrates courteous, helpful and professional behavior.
        • Provides team leader/director with complete and accurate report on patient's condition.

        • Utilizes appropriate communication channels in attempting change.
      • Observes and documents the patient's symptoms, changes and actions.

      • Accurate and timely cross checking of medicine and treatment orders in Cerner and the med carts, and other assigned duties.
        • Daily checking the physician orders.
        • Other duties as assigned might be, i.e., safety check of a room, etc.
      • Performs nursing care as assigned by Charge Nurse, utilizing Policies and Procedures (P/P), Standards of Care and Care Plan. Demonstrates skill in conceptual, technical and procedural nursing practice to include unit specific policies and procedures outlined from orientation checklist.
        • Performs patient care and treatments in a timely manner.
        • Documents care and patient responses following Policies and Procedures (P/P) for documentation.
      • Safely, timely and accurately administers medication.
        • Medication delivery is timely and without error.
        • Watches the patient take their medicine.
        • Observation, communication and documentation of medication effects - to include side effects and positive response.
      • Accurate checking and follow through of physician orders.
        • Physician's orders to be checked within 2 hours of being written.
        • The orders checked completely and without error.
      • Provides timely and accurate observation and documentation.
        • Night rounds include observation of each assigned patient within a 15 minute time period (times being varied so that a patient cannot plan your arrival time).
        • Observations include: sleep patterns, sleep apnea and a more close observation of the patient with suicidal tendencies.
        • Communicates any negative finding immediately.
      • Maintains awareness of the unit as a whole.
        • Keeps self aware of what is happening in all areas of the unit and applies self when and where ever needed according to unit's policy.
      • Provide structure and use nursing intervention for patients who cannot sleep or who have a light night up or is a new admission.
        • Enforce unit policy for those who have a late night up.
        • Provides nursing interventions for those who cannot sleep.
        • Identify patterns and communicate them to the Charge Nurse.

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
