Job description

The Certified Nurse Practitioner in the Nursing Home is the center of the Institutional Interdisciplinary Care Team.

Provide care management, clinical care, and education to the members and their families, as well as education to the staff.

Valor Nurse Practitioners facilitate early diagnosis, intervention, communication, and delivery of the appropriate services in a quality cost effective manner.

This Nurse Practitioner position will serve Cuyahoga County and Lake County area.

PRIMARY DUTIES AND RESPONSIBILITIES:
  • Triages care needs based on the Health Risk Assessment Tool (HRAT) scores. Conducts clinical assessments on an ongoing basis.
  • Provides input into the development of the ICP in collaboration with the case manager and other members of the ICT.
  • When needed, interfaces with other practitioners to help facilitate access to all services (IP, SNF, specialist care, behavioral health, etc.).
  • Visits the patient as scheduled and after hospital discharge to evaluate progress, conduct physical exams, prescribe interventions, and communicate results to staff.
  • Evaluates episodic complaints and initiates treatment in place when appropriate.
  • Obtains consultation and diagnostic reports and reviews as needed with the Medical Director and ICT.
  • Identifies and addresses changes in the members' health status by being available on call when not at the NF during scheduled visits. There is a CNP on call 24 hours a day, 365 days a year to address member needs.
  • Supervised clinically by the Medical Director and operationally by the Care Coordination Director.
  • Advocates, informs and educates beneficiaries and family members on self-management of conditions through regular meetings and informal discussions.
  • Educates Nursing Home staff through ICT meetings and regular communications when on site.
  • Provides in-service education to Nursing Home staff relating to specific needs of the geriatric population (poly-pharmacy, fall prevention, wound care management, etc.).
  • Provides information and documents decisions regarding future care choices, such as the patient's preferences about the use of life-prolonging technologies in the event of a terminal illness.
  • The Certified Nurse Practitioner is the primary point of contact that facilitates communication between the nursing facility, PCP, the beneficiary and/or beneficiary's family and any required specialty care or other social or other services.
  • The CNP will help align performance measures between each link of the care delivery model.
  • The CNP engages the PCP and nursing facility clinical staff in order to develop trust, strengthen communication, and coordinate individual beneficiary's ICPs.
  • In addition to essential position functions listed above, this position would perform other duties as directed by their immediate supervisor.

EDUCATION:

• A valid license to practice as a Nurse Practitioner in the State of Ohio.

o Must not have had a license suspended in any state or have been precluded from participation in any state's Medicaid or Medicare program.

• 3 years' experience working in a health care environment; preferably an HMO/PPO.

TRAINING AND EXPERIENCE:

• 3 years' experience working in a health care environment; preferably an HMO/PPO
  • Management experience preferred


JOB KNOWLEDGE:
  • Basic knowledge of Medicare/Medicaid Regulations
  • Understanding of the healthcare delivery system
