JOB SUMMARY

Promotes and restores residents' health by completing the nursing process; collaborating with physicians and the interdisciplinary teams; providing physical and psychological support to residents, friends, and families; supervising and evaluating assigned team members such as the CNA. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against resident care goals and standards; making or recommending necessary adjustments; following facility and nursing philosophies and standards of care set by the state board of nursing, state nurse practice act, and other governing agency regulations.

Essential Functions
  1. Obtains and provides a nursing report prior to the start of patient care duties.
  2. Communicates the residents needs to the team including the CNA prior to the start of patient care duties.
  3. Makes rounds to observe and evaluate the physical and emotional status of each resident at the beginning of each shift and throughout the shift as needed. Alerts the RCM to resident needs that require immediate attention.
  4. Assists in assigning the direct care of a specific group of residents to the Certified Nursing Assistant based on the needs of the resident and the needs of the facility.
  5. Assures that the Certified Nursing Assistant completes resident care activities in a timely, efficient, and appropriate manner that meets the residents' needs and preferences.
  6. Other duties as assigned (Assists in problem solving of resident care needs, administers medications and treatments in a timely manner and documents as appropriate, performs the narcotic count each shift, documents residents' assessments and treatments as per facility guidelines, supervises and participates in the administration and documentation of dietitian ordered nourishments, supplements and general hydration, etc.)

Education Background
  1. High School Diploma or equivalent
  2. Associate of Nursing Degree.

Licensure, Registration, Certification
  1. Active LPN license in Washington State.
  2. Active CPR and First Aid Certification.

Special Training/Skills
  1. Knowledge of State and Federal regulations as they relate to long-term care facilities.
  2. Demonstrated knowledge and ability to perform clinical responsibilities.
  3. Outstanding communication skills with the ability to speak, interpret, and understand conversational English.
  4. Computer skills to include experience in MS Office (Word, Outlook) and Electronic Health Records systems.
  5. Maintain professional competency by independently attending continuing educational offerings.

Work Experience
  1. Previous Nursing experience in a Skilled Nursing Facility.
  2. Experience with IV's, maintaining pain infusion pumps, and feeding tubes.
  3. Experienced in supervising a staff of multiple employees.

Physical Demands
  1. Ability to stand/walk 4 - 6 hours per day.
  2. Ability to sit for long periods of time for charting, phone consulting, data entry.
  3. Ability to lift up to 50lbs on occasion.
  4. Ability to bend, stoop, and squat frequently for assessment of residents.
