NOTE: This role is based in office M to F and 9 to 5. ONLY LOCAL TAMPA BAY CANDIDATES WILL BE CONSIDERED.

This role offers an exciting opportunity to work in a cutting-edge environment, shaping the future of healthcare by providing care to patients who are at home/in a non-office setting via telehealth and remote patient monitoring (RPM) to achieve optimal health outcomes. Specifically, the Telehealth LPN will service a panel of patients and provide virtual health and wellness coaching based on the interpretation of health vitals. This is an excellent opportunity to work with a dynamic, multidisciplinary team of Clinicians including Doctors and Nurse Practitioners dedicated to enhancing remote patient care.

Responsibilities

• Actively monitor patient clinical data signs and meet via audio or video calls to discuss condition management

• Communicating effectively and regularly with patients who are being monitored to ensure a quality patient experience

• Ability to effectively document and notate patient communications and relating interactions and/or interventions

• Understanding and escalating patient care based on established protocols

• Adapts easily to changes in work responsibilities

• Educate patient on the meaning of their health vitals

• Educate patient on preventative care options which promote health and well being

• Answer, direct, and follow up on incoming calls

• Utilizes standardized treatment approach to provide high level of care to patients

Education

• Licensed Practical Nurse

Experience

• At least 5 years working in a medical office or hospital ward which managed pulmonary and cardiac patients

Skills

• Good understanding of normal blood pressure and oxygen and heart rate measurements

• Individual must demonstrate professionalism and have excellent telehealth including telephone skills

• Prioritization and time management between multiple activities

• Excellent written and verbal communication skills

• Understanding of HIPAA privacy laws

• Ability to perform patient assessments based on data and communication without physical examinations

• Ability to comprehend and analyze vital sign data

• Understanding of virtual patient meeting etiquette

• Proficiency utilizing Windows-based computer applications, email, and web-based apps

• LPN - Licensed Practical Nurse - Florida State Licensure
