Join the team of health care professionals who provide services to our residents at The Alton, a 65 unit memory care residence. The property is located in St. Paul's Highland Park area and is part of StuartCo's own Shepard Park neighborhood of seven rental lifestyle communities. The location is easily accessible via I-35E, I-494, Highway 5 and Crosstown 62 -- you can get here from just about anywhere! The members of our LPN team provide direct care to those in need of such services.

Monday through Friday -Days (No weekends) - shift available!

Hiring Bonus:

• Day or Eve Shifts at The Alton $2,000.00 Hiring bonus with $1000 paid on first available pay date following hire date.

Example: If start date is 7/16, $1000 paid on 7/31 and $1000 paid on 8/15.

Job Objectives:

This position will ensure that the health and safety of all residents are met. The LPN will assist the RN with planning, coordinating, and managing the assisted living services and act within the Assistive Living Licensure regulations. The LPN is responsible for the overall direction, coordination, and monitoring of assisted living services delivery under the direction of the Director of Health Services.

Supervision (Given and Received)
  • Given: In conjunction with their Essential Job Functions and Tasks.
  • Received: The LPN works under the direction of the Director of Health Care Services (RN). The employee is responsible for the method in which to complete their work.

Qualifications:
  • Must be licensed with the State of Minnesota as a LPN, and must maintain current and unencumbered licensure.
  • CPR certification required.
  • At least 1-year experience in related field.

Other Specialized Knowledge and Abilities:
  • Possess a sensitivity and aptitude for working with the elderly.
  • Possess skills to communicate effectively with those you supervise, residents, family members and RN Supervisor.
  • Possess leadership / supervisory skills.
  • Must be able to prioritize and organize work effectively and efficiently.
  • Able to work irregular hours, occasional weekends and evenings.


Essential Job Functions and Tasks:

Job Function: Work within the program guidelines including completing essential paperwork and complying with state regulations.

Tasks:
  • Works to assure facility is in compliance with federal, state and local standards and regulations.
  • Maintains an up-to-date knowledge of current trends of Assisted living.
  • Works to maintain privacy and confidentiality of resident, resident's records and resident living environments.
  • Is willing to carry a cell phone and be available for staffing and notifications.
  • Communicates and documents resident data to staff as needed.
  • Assures residents maintain residency requirements, monitors and recommends alternative placement if necessary.
  • Reviews resident service agreements with RN, as needed.
  • Monitors and maintains resident charts and nursing notes.
  • Communicates to physicians any changes in resident's needs or conditions, as needed.
  • Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.
  • Fills weekly pillboxes for medication administration by unlicensed staff and checks new medications from the pharmacy.
  • Communicates in staff communication book any information that staff needs to be aware of.
  • Assists with compliance in meeting regulations and is available for state licensure surveys.


Job Function: Provide support and address needs for programs and services.

Tasks:
  • Maintain resident files and appropriate documents as required.
  • Monitors residents on an ongoing basis to assure their needs can be met.
  • Assures residents continue to live safely in their unit, monitors and recommends alternative care and services, if necessary.
  • Inform personnel of changes in resident's needs and conditions.
  • Assists ULP in service delivery.
  • Maintains communication with residents, families, and other staff.
  • Responds to resident concerns or complaints.


Job Function: Participate and function effectively as a team member.

Tasks:
  • Develop and maintain a positive working relationship with staff of StuartCo, including providing backup to other staff as needed.
  • Articulate information regarding residents to the team.
  • Provide proactive, constructive participation in staff meetings, committees, etc.
  • Assists other departments with special events and family functions.
  • Be compassionate and work with tact and ethical awareness.
  • Be flexible and adaptable to changing situations.
  • Responds appropriately to safety hazards, fire drills and other emergency situations.
  • Reports injuries for self, staff, residents or visitors immediately to RN Supervisor.
  • Keeps RN Supervisor informed and involved and advised of needs and problems.

Job Function: Other duties as assigned.
