Duties SummaryThis position is eligible for the Education Debt Reduction Program, a student loan reimbursement program. You must meet specific individual eligibility requirement, and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office.Recruitment/Relocation incentive may be authorized to a highly qualified candidate.Responsibilities
A Palliative Care consultant is a physician who is responsible for improving the palliative medical care delivered by the Palliative Care Consultation Team (PCCT) at the Louis A. Johnson VA Medical Center with outpatient experience and Hospice & Palliative Medicine (HPM) experience.
Duties include but are not limited to:
Serve as the facilities GEC (Geriatric and Extended Care) Coordinator.Provide physician services, clinical input and oversight for all patient treatment plans consistent with hospice and palliative care philosophy.Improve access to Hospice and Palliative care as experienced by patients, their families and providers.Coverage of inpatient palliative care consultations and in-patient palliative care and hospice admissions.Coverage of outpatient palliative care clinic (including Face 2 Face, Telephone and VVC clinics).Participate in Staff education, and coordination of hospice and palliative care at the Louis A. Johnson VA Medical Center.Amplify the awareness of the concepts, role and availability of Hospice and Palliative Care services among ambulatory, home-based, specialty and acute care clinicians.Facilitate access to appropriate supportive care services.Consult with attending staff as requested so that symptom management for patients is effectively and expeditiously accomplished.Complete full and appropriate documentation of patience care services.Actively contribute to the educational experience for medical students, residents, fellows and all other staff assigned to Hospice and Palliative Care.Collaborates with other VA Palliative Care leaders, VISN leadership, and VA Central office on program development issues.Updates, monitors and promotes compliance with VA regulations to include updates for changes in applicable VA memos, directives and guidelines.Collaborates with community home health and hospice agencies as needed for Palliative Care patients currently under his/her care.Maintain an active license to practice Medicine and all related credentials within the Louis A. Johnson VA Medical Center.Attain and provide information about federal, state, and local regulations and codes applicable to Hospice and Palliative Care programs, applicable to the Joint Commission (JC), and the professional service and administrative requirement and expectations of third-party payers.Improves patient outcomes through the implementation of strategies such as clinical practice guidelines and critical pathways, health risk assessments, provider profiling use of electronic medical record and clinical reminders, and case management.Effectively communicates with service employees, patients and their families, employees from other services, stake holders, the Executive Leadership team, the VISN and VACO staff when required.Other duties as assigned.Work Schedule: Monday-Friday, 8:00 A.M. - 4:30 P.M.; however, subject to change, depending on the needs of the service/medical center. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.Proficiency in spoken and written English.Physical requirements outlined below.Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office.Physical Requirements: Lifting and carrying (up to 45 lbs.); pushing or pulling (1 hour); reaching above shoulder; use of fingers; both hands required; walking (up to 4 hours); standing (up to 4 hours); repeated bending (up to 2 hours); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other eye; hearing (aid permitted); clear speech; mental and emotional stability.
