This practice sees patients in facilities (ALFs/ILFs) and private residences throughout Indianapolis and the surrounding communities. The practice works with providers to define territories that are mutually beneficial and ensure comfortable commute times.

Position

DESCRIPTION:

The physician will serve as a primary care provider for a small, dedicated panel of geriatric patients.

The focus is on preventative medicine, chronic disease management, and routine primary care.

The office staff performs all scheduling duties, answers and triages phone calls, and supports the physicians however possible.

Physicians see 8 to 12 patients per day, efficiently scheduled by the office team.

Patients are seen as often as deemed necessary by the physician, which is typically 1 visit per month.

Physicians utilize a mobile EHR on a laptop.

There are no evening or weekend responsibilities. On-call is flexible/negotiable - phone triage only with low volume.

Requirements

:

* Indiana medical license
* DEA
* Board Certified or Eligible, relevant field
