Job Description

PACE of Southwest Michigan is seeking a Primary Care Physician to add to our team.

The Primary Care Physician (PCP) provides all aspects of direct care to all participants of the program. The PCP works under the direct and indirect supervision of the Medical Director or designee but is independent in the application of advanced physician knowledge and skills.

Education: Graduate of an accredited School of Medicine, Residency in Internal Medicine or Family Practice. Board certified in internal medicine or family practice preferred. Additional training/Fellowship in Geriatrics preferred.

Experience: At least 2-3 years' experience.

Number and Type of Employees Supervised (optional): None, except clinical supervision of nurse practitioner staff.

Licensure, Registry or Certification Required: Current unencumbered license to practice medicine in MI, Current unencumbered DEA license, valid MI driver's license and vehicle.

Performs comprehensive history and physical on new referrals.

Performs interval history and physicals on established participants completed quarterly or as indicated.

Assists in the development, implementation and evaluation of the Plan of Care

Evaluates and treats participants with episodic illnesses.

Refers participants to medical specialist as indicated.

Maintains active hospital privileges: (a) Provides primary care consultation to the ER and hospital team for the participant management; (b) Provides updates to the team on a regular basis; (c) Judiciously utilizes specialty consultants; (d) Responsible for coordinating discharge planning with the team.

Manages all care of participants in the nursing home: (a) Performs telephone contacts with nursing home staff as required; (b) Admits nursing home participants to hospital when necessary.

Regulates and adjusts participant's medications as needed.

Educate participants and families on preventive health maintenance of chronic medical conditions.

Actively participates in family conferences.

Demonstrates active participation in Quality processes

Provides supervision for mid-level staff.

Documents participant changes appropriately in the medical record and communicates participant changes to team members.

Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.
