- MSN/ APRN (Advanced Practice) / ARNP/ FNP/ - Family Nurse Practitioner/ NP , Adult Nurse Practitioner / ANP , Pediatric Nurse Practitioner / PNP or Physician Assistant / PA
- License and Board Certification in state of NY
- Spanish fluency is required for this role if a new PA
