House Calls NPs deliver high-touch, high-quality primary care to our homebound patients. This role is supported by a scribe/medical assistant, nurse, social worker, and other resources. NPs make 6-7 visits daily in a defined territory and collaborate continuously with other team members for care coordination and all other needs.

- 30-45 minute assessments
- Mileage is reimbursed
Job Description:
House Calls NPs deliver high-touch, high-quality primary care to our homebound patients. This role is supported by a scribe/medical assistant, nurse, social worker, and other resources. NPs make 6-7 visits daily in a defined territory and collaborate continuously with other team members for care coordination and all other needs.
This job will have the following responsibilities:
- Responsible for conducting in home visits on assigned patients
- Patient education regarding health status
- Collaborate with other medical professionals to ensure quality care
- Follow designated care plan and changing as medically necessary
- Additional duties as needed
Qualifications & Requirements:
- Must have an active PA Nurse Practitioner license in good standing
- Experience in home-based care (as RN or APN)
- 1+ years APN experience, preferably with a Medicare population
- Bilingual in English/Spanish preferred
- Must be willing to travel as required
