Visiting LVN's Needed for Growing Home Health Agency

As a result of our contracting efforts, we have a NON-STOP MAJOR INFLUX OF PATIENTS.

-Home Health Experience Preferred-Ability to utilize Electronic Charting System (Kinnser)

-Assessment Skills Required

-Coordination of Care among different disciplines.

Compensation is $35 - 40 per visit for employees depending on experience/knowledge
We have a very good support staff and our Director of Nursing is very knowledgeable and hands on. This is a team effort.

The spectrum of care provided ranges from medication teaching to wound vac, blood draws, etc.

The need is immediate and our hiring process is simple (less than 1.5 hours).
We utilize a web-based software called Kinnser / WellSky to simplify the documentation process.

We pay weekly.

Experience in home health visits is a plus.
Compassionate and loves to listen to / educate patients.
Proficiency in computer/ smartphone for electronic notes use.
Knowledge of Kinnser/Wellsky software is preferred (but not required).
Willing to grow with our company.
Bilingual is a plus.

Can work INDEPENDENTLY without the need for supervision.

Job Types: Full-time, Part-time

Pay: $35.00 - $40.00 per hour

Job Requirements:

• Hire select home health care personnel
• Provide nursing guidance and care to patients at home
• Direct the home care patient's care based on individual patient needs
• Performing in-home visits for home health clients
• Coordinate care with other health care professionals
• Give accurate information to patients, patient families and home health care staff members regarding cost for home health care services
• Supervise home health aides and licensed practical nurse in care
• Serving home health patients
• Provide professional nursing care for assigned patients
• Provide a wide range of nursing care to patients in the home care setting
• Administer skilled nursing care to patients requiring intermittent professional nursing services
• Assess the patient's condition during every home health care visit
• Work in a home setting providing care to home bound patients
• Conduct regular patient care conferences and coordinate patient care with all care providers
• Coordinating nursing care for patients based on established clinical nursing practice standards
• Provide a wide range of nursing care to patients in the home care setting
• Provide nursing care to patients in the home setting
• Ensure market development and growth
• Administer skilled nursing care to patients requiring intermittent professional nursing services
