We are seeking multiple Nurse Practitioner / Physician Assistant - Primary Care to join our team! You will provide healthcare services, under the supervision of a physician.

Responsibilities:

• Administer injections, immunizations and other medical procedures
• Assess patient health through interviews and physical examinations
• Provide counsel and medical treatment to patients
• Educate patients on overall wellness and health maintenance
• Develop and implement patient management plans
• Maintain a safe and clean work environment

Qualifications:

• Previous experience as a nurse practitioner or physician assistant in primary care
• Fully licensed in AZ inc. DEA (in good standing)
• Familiarity with medical tools and equipment
• Ability to build rapport with patients
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills

Home health providers are currently preferred and will be working in a 20 mile radius of their location, tele health providers will be working out of our office in Surprise and starting from March, 2022 in Peoria.
