Regional or StateTravelers wanted
Nurse Practitioner, Physician Assistant, orPhysician MD/DO Health Risk Assessments
Currently looking for Part-time or full-timeBoard-Certified Nurse Practitioners, Physician Assistants, or Physicians MD/DOto conduct Health Risk Assessments on Medicare Advantage members. Experience incritical care, emergency medicine, adult physician practice, or hospice/homecare a plus but not necessary.
Perform in-home wellness assessments withMedicare recipients to pinpoint any gaps in care. Assessments cover priormedical history, present history, and medication review. Educate members onchronic health conditions, medications, preventative screenings/tests, andlifestyle concerns.
Providers are paid weekly per completedassessment as 1099 independent contractors
Pay: $2000 - $3200 / per weekdepending on availability-incentive pay based on monthly hourlycommitment
Responsibilities of the Nurse Practitioner, PhysicianAssistant, or Physician MD/DO - Health Risk Assessments**
• Conduct comprehensive in-home health riskassessment to identify all active and chronic disease conditions as well asdetermine all physical, mental and social needs present at the time of thevisit.
• Obtain vital signs, measure BMI, reviewpharmacological therapy and conduct a physical examination.
• Educate and counsel patient and family on anyconditions identified during assessment and screening procedures.
• Make recommendations and communicate findingsto Primary Care Physicians through documentation of outcomes on providedtablet.
