Job Description
Advanced Practice Registered Nurses (APRN) will provide primary care and maintain continuity of care within the Family Medicine Department at Premier Community HealthCare Group (PCHG). We strive to serve in establishing diagnosis and treatment of common acute and chronic illnesses with health promotion and disease prevention to promote health.

Essential Duties and Responsibilities:

• Assure comprehensive healthcare needs are identified and treated and the people served are educated about their illnesses and given health management information and techniques.

• Communicates effectively with all members of the Care Team to ensure smooth handoffs and a positive patient experience.

• Consults and coordinates with health care providers working at any location.

• Works in coordination with the Quality Assurance Team and management to develop and implement a quality assurance program.

• Participates in a rotation for afterhours services on-call

• Manage daily and weekly schedule to ensure encounter targets are met and documented appropriately in the Electronic Health Record (EHR)

• Review and address tasks daily and maximize productivity.

• Ensure confidentiality and HIPAA compliance at all times

• Keep other care team members informed when situations occur that disrupts timely patient flow through site.

• Adhere to best practice in pediatric medicine and the core values of Premier Community HealthCare to provide a best-in-class patient experience

• Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA)

• Other duties as assigned

Supervisory responsibilities: None

Knowledge, Skills and Abilities:

• Solid understanding of Pediatric medicine

• Ability to effectively evaluate and identify diagnosis within children

• Identify medical history

• Exceptional customer service skills

• Ability to communicate effectively, verbally and in writing with patients, care givers, and multiple levels within the organization.

• Excellent grammar, spelling and interpersonal skills.

Qualifications:

• Master's degree in an accredited Nurse Practitioner program with a clinical specialty area of Pediatric Medicine

• Current Florida Advanced Registered Nurse Practitioner (APRN) license.

• Prefer three (3) years of experience as a Nurse Practitioner

• Current Basic Life Support (BLS) certification.
