Hope Community Resources is a non-profit organization providing community supports to hundreds of individuals and families who experience intellectual and developmental disabilities, traumatic brain injury and mental health challenges. The people who choose our supports range in age from infancy to the elderly, are of diverse ethnic background, and experience a range of disabilities. The Home Based Nurse provides services to Hope support recipients through their work in supervising the health care management of support recipients who experience intellectual and/or developmental disabilities and who experience complex medical conditions.
Ensures proper and consistent care, manages medication provision, and executes physician's orders.Coordinates activities with other staff members to promote optimum health and wellness for support recipients, and evaluates support recipient health and psychosocial status on a regular basis to detect health complications or medical emergencies. Utilize professional experience and knowledge to function independently, providing direction and education to staff and families.
SCHEDULE DETAILS: Thursday-Sunday, 10PM - 6AM
