Overview

Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has over 25 primary providers including: physicians, nurse practitioners, physician assistants, physical therapists, and a marriage and family therapist. With a professionally trained staff of CMA's, receptionists, lab techs and others, FPA provides award winning care that has been recognized as a Patient-Centered Medical Home (PCMH) through the National Commission on Quality Assurance. FPA works diligently to offer Central Kentucky families enhanced access to medical care at its Hamburg & Brannon Crossing locations as well as online at www.fpalex.com.

Responsibilities

The mid-level provider is responsible for providing professional level primary health care to patients with complex chronic and acute health conditions under the direction of a collaborative provider (if required). They provide general and preventative care, conduct check-ups, treat illnesses, order lab tests and prescribe medication for children and adults (within scope of practice)

  • Obtain/Review detailed Medical History
  • Perform Review of Systems
  • Perform Physical Exams
  • Assess medical problems
  • Diagnose and manage patient care
  • Order and interpret test results and recommend treatment plan
  • Complete EHR documentation
  • Maintain other patient documentation as required
  • Provide patient education to patients
  • Perform other medical or educational functions as indicated in the scope of practice for a Nurse Practitioner or Physician Assistant, which has been authorized within the assigned working hours
  • Comply with all State Laws governing Nurse Practitioners or Physician Assistants
  • Practice safety, environmental, and/or infection control methods (follow FPA's guidelines, insure Medical waste is properly disposed, maintain a safe and clean work environment)
  • Analyses new knowledge gained from conferences, workshops, professional literature and assimilates the knowledge into the practice
  • Any other tasks deemed appropriate and assigned by FPA


Specific Requirements
  • Licensed as Nurse Practitioner or Physician Assistant
  • Knowledge of primary care principles and practices
  • Thorough understanding of patient care, community resources, drugs and their indications, contraindications, dosing, side effects, and proper administration
  • Ability to perform medical examinations through observations and assessments using standard medical procedures and record all symptoms, reactions ad progress
  • Ability to react calmly and effectively in emergency situations
  • Ability to educate patients and or families and to provide instruction on proper care and treatment and maintain detailed records
  • Ability to establish and maintain positive, professional relationships with all peers, management and patients
  • Computer literacy: Fine motor skills necessary to operate the computer
  • Physical demands: Ability to stand up most of the day; walk, sit and stand up frequently; stretch repeatedly with arms and hands; use hands and fingers to conduct various medical procedures, physical exams, diagnostic evaluations and computer work; lift and/or move loads up to 20 pounds; vision abilities - close vision, distance vision and ability to adjust focus; must possess hearing and speech to communicate in person and over the phone, able to focus in an environment with many distractions
  • Communication: Good communication skills (written and oral)
  • Knowledge of Electronic Health Record preferred
  • Problem solving ability, independent judgment and initiative
  • Ability to prioritize and develop workflow


Must be willing to travel between locations (Hamburg and Brannon Crossing)
