Position Summary:
The Supervisor of RN Care Coordination, under the supervision of the Regional Manager, Care Coordination Clinical Services is responsible for planning, organizing, and coordinating daily operations of the nurse care coordination across Health groups New Jersey locations to ensure quality patient care and outcomes for highest-risk patients.

Essential Job functions:
  • In collaboration with the Care Coordination Management team, creates and implements policies and procedures, workflows, processes, and emergency protocols.
  • Manages a caseload of 20-30 high risk / segment 4-6 patients.
  • Supervises a team of Nurse Care Managers to ensure that all documented activities meeting evidence-based practice standards, regulatory / payor requirements, and internal policies and procedures.
    • This may be inclusive of reviewing utilization management metrics, discharge planning functions and performance improvement activities.
  • Oversees weekly performance targets and provides 1:1 coaching, education, and support to Nurse Care Managers.
    • Responsible for monitoring weekly encounters and caseload sizes
    • Review key clinical performance indicators such as ED and IP utilization
    • Assesses Provider / Nurse Care Manager collaboration
    • Identifies and documents opportunities for performance improvement
    • In collaboration with the QA/QI Specialist conducts and / or evaluates random chart sampling of Nurse Care Manager
  • Holds weekly team huddles to review workflows, policies and procedures, and departmental communications such as education, training, etc.
  • Provides support to payor clinical collaborations including creation of agenda, attendance at meetings and responsible for follow-up or escalation to Manager.
  • Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery.
  • Assists with difficult, unusual, or emergency situations to minimize financial, legal or risk management liability for the facility. Communication with patients and their families around medical plan of care and discharge plan, in difficult cases.
  • Create and support an environment that fosters teamwork, transparency, cooperation, respect, and diversity.
  • Act as a care coordination resource to other departments regarding the role of care coordination in team-based care.
  • Supervises and signs off on time and attendance procedures.
  • Identify and report potential risk(s), operational opportunities, and barriers, with the ability to problem solve and recommends solutions to and with the leadership team.
  • In collaboration with Regional Manager, completes annual performance review for Care Coordination staff according to SH policy and provides ongoing feedback.
  • Plan, develop and conduct educational workshops for staff members to improve skills, aid in professional growth and development.
  • Plans and assists in the hiring and orientation of new office staff.
  • Supports Career Ladder opportunities to assist staff to meet their highest potential.
  • Monitors departmental compliance with all applicable standards (OSHA, HIPPA, Occurrence reporting); reports compliance concerns; ensures utilization of resources for resolution of issues.
  • In collaboration with Clinical Manager, advances the department's technology including the use of information technology and the implementation and appropriate use of the Electronic Medical Record, Departmental/Ancillary Systems.


Competencies:
  • Experience and proficiency in chronic, complex, transitional care management and care coordination.
  • Maintains core proficiencies and competencies as defined by the Care Management Program; including, but not limited to, motivational interviewing, strong assessment skills, critical thinking, patient education, care planning, collaboration, and ability to build trusting relationships.
  • Continually seeks new knowledge and learning regarding comprehensive primary care and chronic disease management principles.
  • Act as support team member in assigned departments to assist in patient care activities as needed in high volume situations.
  • Other duties as required.


Physical Job Requirements:

• Physical strength to lift heavy objects, carts, or items, taking frequency and weight into consideration.

Blood-borne Pathogens:
  • Contact with patients or patient specimens are possible.


Education, Certification, Computer and Training Requirements:
  • BSN Degree required.
  • Valid NJ RN License and valid driver's license required.
  • Certified Case Manager (CCM) preferred (If you do not have this, they will need to obtain it within 6 months).
  • Minimum 4 years clinical patient care experience in acute care setting and chronic disease management required. Minimum 2 years case/utilization management experience in an acute care or insurance company setting preferred.
  • Experience supervising teams of clinical and non-clinical staff such as nurses and patient navigators
  • Ability to function independently, self-starter required.
  • Ability to communicate in English, both orally and in writing required.
  • Ability to use initiative and judgment when making decisions required.
  • Experience with Microsoft Office applications including Word, Excel, Access and Email/Internet required. Ability to work with data files required.
  • Experience with Standard Office Technology in a Window based environment required.
  • Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Ability to effectively work remotely and in office environments.


Travel:
  • As needed between locations for job requirements.
